I certify that all information provided, including health history, allergies, and medical conditions, is true, complete, and accurate to the best of my knowledge. I understand that it is my responsibility to inform the school of any changes. - Medical Care & Emergencies
In the event of an emergency, I authorize the school to seek medical treatment for my child if I cannot be reached. I understand that any medical expenses incurred are my responsibility. - Allergies & Medical Conditions
I acknowledge that while the school will make reasonable efforts to support students with allergies or medical conditions, it cannot guarantee an allergen-free environment. - Health & Illness Policy
I agree to keep my child at home when they are ill and to follow the school’s health, safety, and communicable disease policies. - Behaviour & Discipline
I understand that my child is expected to follow the school’s code of conduct, including expectations related to safety, respect, and appropriate behaviour. The school reserves the right to implement disciplinary measures when necessary. - Attendance & Participation
