PHOTO CONSENT : I give permission to Chardikala Khalsa Camp to take and use photographs, videos, or audio recordings of my child(ren) for camp-related activities, promotions, and future publicity purposes. MEDICAL AUTHORIZATION I have provided Chardikala Khalsa Camp with complete medical information for my child(ren) and can be reached at the contact number(s) listed. I authorize camp staff to provide first aid or seek necessary medical treatment for my child(ren) in case of an emergency, following the recommendations of medical professionals. ACTIVITY CONSENT & LIABILITY WAIVER I understand that my child(ren) will take part in various activities at school sites and other locations during Chardikala Khalsa Camp. I accept that participation carries certain risks, and I agree that Chardikala Khalsa Camp, including its staff, volunteers, and affiliated organizations, cannot be held responsible for any injury, loss, or damage that may occur to my child(ren) or their belongings during the camp.
